Conversion Form and Guidelines – used to earn Honors credit for a non-Honors course

Departmental Consent Form – Part of the application package for rising juniors who would like to join the Honors Program.

Faculty Evaluation Form – Part of the application package for rising sophomores and juniors who would like to join the Honors Program.

Final Plan of Study – This form must be submitted before a student can graduate as an Honors Scholar.

Preliminary Plan of Study – Part of the application package for rising juniors who would like to join the Honors Program. Other honors students are encouraged to use this as a tool to work out their junior/senior honors requirements.

Thesis Approval and Cover Sheet – This form must be submitted along with the final copy of a student’s thesis.

Thesis Plan – This form must be submitted before a student can graduate as an Honors Scholar.
